Job Description

Job Responsibilities

Clinical Reviews and Input

  • Reduce GP time spent on patient appointments by handling clinical medication reviews, urgent medication requests, and queries.
  • Support patients with their medications, ensuring optimal use.
  • Manage minor ailments triage and patient caseloads.
  • Provide patient-facing clinics to improve medication adherence.
  • Conduct multidisciplinary reviews on medication-related issues.
  • Reconcile medicines post-hospital discharge.
  • Review daily pathology results for selected patients.
  • Undertake targeted medication reviews, especially for high-risk patients.
  • Administer influenza and travel vaccines.
  • Support the management of patients with drug dependence and alcohol-related disorders.
  • Signpost patients to appropriate services and information sources.

Unplanned Admissions and Discharge

  • Review and audit medicines associated with unplanned hospital admissions and readmissions.
  • Implement changes to reduce high-risk medicines prescribing.
  • Reconcile medicines following discharge and ensure continuity of supply for high-risk groups.

Medicines Optimization

  • Assist with achieving QOF targets for long-term conditions management.
  • Undertake training in health assessments, history taking, and physical examination.
  • Commit to starting an independent prescribing course within six months of probation.
  • Reduce inappropriate poly-pharmacy.
  • Address medicines adherence with follow-up calls.

Audits and Risk Stratification

  • Conduct regular K&W audits and practice-specific audits to enhance prescribing quality and safety.
  • Implement drug withdrawals, alerts, and NICE guidelines.
  • Identify high-risk patient cohorts through practice computer searches.

Repeat Prescribing Process

  • Improve communication with community and hospital pharmacies about discharges and repeat prescribing.
  • Enhance the repeat prescribing process within K&W practices.
  • Implement and manage the repeat dispensing process.
  • Synchronize patients’ medications to reduce wastage.
  • Ensure all practices have a repeat prescribing policy.

Medicines Management

  • Improve prescribing quality, safety, and cost-effectiveness by liaising with GPs and other prescribers.
  • Provide medicines information to K&W practice teams and patients.
  • Ensure practices comply with CQC standards where medicines are involved.

Public Health

  • Support public health campaigns and provide specialist knowledge on available programs.
  • Maintain an open-door policy for medication advice and urgent issues.

Other Duties

  • Assist with implementing local and national frameworks and strategies to improve patient care.
  • Work with other professionals to ensure a streamlined service delivery.
  • Contribute to new initiatives and ways of working to maximize service potential.
  • Maintain professional practice and development, ensuring compliance with regulatory bodies.
  • Educate callers on correct service use and health-related organizations.

Person Specification

Qualifications

Essential

  • Master of Pharmacy (MPharm) degree
  • One-year pre-registration training course in pharmacy
  • Accredited by the General Pharmaceutical Council (GPhC)
  • Relevant A-level and GCSE qualifications (grades A to C)

Experience

Desirable

  • Ideally, 2 years of experience in General Practice

UK Registration
