Job Summary

Main Duties of the Job

As a Clinical Pharmacist, you will play a critical role in providing expert pharmaceutical care, ensuring the safe and effective use of medications. Working alongside a multidisciplinary team, your responsibilities will include:

  • Managing patients with co-morbidities proactively.
  • Supporting general practice staff with prescription and medication queries.
  • Overseeing the repeat prescriptions system, handling acute prescription requests, and managing medicines reconciliation during care transitions.
  • Leading on medicines optimization and quality improvement initiatives.
  • Conducting clinical triage and managing the GP practice appointment book.

Job Description

Responsibilities

  • Develop and manage a comprehensive medicines management plan.
  • Lead the medicines management team and deliver training to maximize cost-effective prescribing.
  • Attend site visits punctually to meet contractual obligations.
  • Perform audits to ensure clinical appropriateness and regulatory compliance.
  • Communicate effectively with patients, families, and staff.
  • Undertake medicines management audits on-site.
  • Utilize digital tools and software provided by the ICB.
  • Provide specialist information and advice to healthcare professionals.
  • Conduct clinical training seminars.
  • Support policy reviews and changes as necessary.
  • Maintain registration with the General Pharmaceutical Council.

Skills (Essential)

  • Ability to work independently and manage time effectively.
  • Excellent attention to detail and accuracy.
  • Commitment to clinical knowledge development.
  • Basic IT skills (e.g., Microsoft Word, Excel, PowerPoint).
  • Strong written and verbal communication skills.
  • Aspiration for clinical excellence.

Qualifications (Essential)

  • Master’s degree in pharmacy (MPharm, B.Sc., or BPharm).
  • GPhC Registered.
  • Independent Prescriber.

Knowledge (Essential)

  • Understanding of General Pharmaceutical Council Codes of Professional Conduct, Ethics, and Performance.
  • Knowledge of the Care Standards Act and Care Quality Commission Requirements.

Experience (Essential)

  • Previous experience working in General Practice.
  • Proficiency with Emis Web.
