Summary
The NHS is seeking a dedicated Clinical Pharmacist to join our multidisciplinary team in Middlesbrough. This role involves working closely with other healthcare professionals to provide patient-centered care, with a focus on medication management and optimization.
Primary Duties and Areas of Responsibility
- Patient Assessment and Treatment: Utilize your expertise in medicines for specific diseases to clinically assess and treat patients.
- Prescribing and Care Management: Serve as a prescriber and manage care for patients with chronic diseases, conducting clinical medication reviews especially for the elderly and those with complex medical conditions.
- Medication Expertise and Public Health: Offer specialist knowledge on medication use to address public health and social care needs, contributing to reducing health inequalities.
- Leadership in Medicines Optimization: Lead initiatives for medicines optimization, ensuring the conservation of antibiotics and improving overall medication use quality.
- Structured Medication Reviews: Conduct reviews to help patients manage their medications effectively, reducing waste and promoting self-care.
- Healthcare Team Integration: Support the integration of general practice with broader healthcare teams to enhance patient outcomes and access to care.
- Collaboration and Relationship Building: Work closely with other pharmacy professionals and healthcare providers to improve patient care and system efficiencies.
- Clinical Protocols and Research: Play a central role in shared care protocols, clinical research, and collaboration with specialist pharmacists.
- Professional Network Participation: Engage in a professional clinical network and undergo appropriate clinical supervision.
Collaborative Working Relationships
- Communication and Cooperation: Use effective communication to gain cooperation from patients, colleagues, and other stakeholders.
- Personal Limitations and Referrals: Recognize when to refer patients to more appropriate healthcare providers.
- Effective Working Relationships: Develop and maintain strong working relationships within and outside the practice.
- Liaison with Stakeholders: Collaborate with the CCG Medicines Optimization Team and other stakeholders to ensure consistent patient care.
Knowledge, Skills, and Experience Required
- Experience with Long-Term Conditions: Experience with common long-term conditions in a general medical practice setting.
- Qualifications: Minimum of 3 years post-graduate experience in pharmacy and working towards or holding an independent prescribing qualification.
- Pharmaceutical Care Management: Ability to manage and review pharmaceutical care programs across core disease areas.
- Professional Expertise: Accountable for providing professional expertise and direct service provision.
- Problem-Solving Skills: Able to prioritize and address deviations from normal patterns, referring to GPs/ANPs as needed.
- Policy and Procedure Compliance: Follow legal, ethical, professional, and organizational policies and procedures.

- Governance and Vision: Understand and implement pharmacy role in governance and contribute to workplace vision.
- Quality Improvement: Strive to improve service quality within set limitations.
