Summary
As a Clinical Pharmacist at NHS Jobs, your role involves a comprehensive range of responsibilities to ensure the optimal treatment and management of patients. Key duties include:
Monitoring, advising, and optimizing treatment for patients with long-term conditions such as diabetes, asthma, hypertension, and COPD.
Managing and authorizing repeat prescriptions, ensuring continuity and adherence to treatment plans.
Conducting thorough medication reviews to enhance patient safety and efficacy.
Providing patients with informed advice and guidance on the effective use of medicines.
Offering guidance and treatment based on blood results in alignment with practice protocols.
Providing appropriate advice and treatment in line with NICE guidance for acute or ongoing medical needs.
Utilizing clinical judgement to assess acute prescription requests promptly.
Diligently processing medication changes from discharge notes and outpatient letters.
Playing a pivotal role in developing and improving practice standards related to medicines management. This includes conducting audits, producing protocols, and providing guidance to the wider practice team.
