Summary: As a Clinical Pharmacist at NHS Jobs, you will be an integral part of a dynamic, multi-disciplinary team, working in a patient-facing role. Your responsibilities will include managing areas of chronic disease, conducting clinical medication reviews, and proactively handling patients with complex polypharmacy. You will provide primary support to general practice staff, addressing prescription and medication queries, managing the repeat prescriptions system, and handling acute prescription requests.

Your role extends to medicines reconciliation during care transitions, implementing systems for safer prescribing, and offering expertise in clinical medicines advice. Your focus will also encompass public and social care needs within the GP network practices. As a Clinical Pharmacist, you will provide clinical leadership in medicines optimization and quality improvement, overseeing aspects of the quality and outcomes framework, as well as enhanced services.

Key Responsibilities:

  • Play a pivotal role in integrating with community and hospital pharmacy to optimize skill mix, improve patient outcomes, ensure better access to healthcare, and manage workload effectively.
  • Provide leadership on person-centered medicines optimization, aligning with local antimicrobial stewardship guidance to conserve antibiotics.
  • Manage some aspects of the quality and outcomes framework and enhanced services, ensuring compliance with guidelines and standards.
  • Undertake structured medication reviews (SMRs) and follow-up consultations, adhering to detailed guidance and using appropriate clinical decision-making tools.
  • Clearly record all SMRs within GPIT systems, utilizing appropriate clinical codes to signify the reasons for an SMR.

Requirements:

  • Qualified and registered pharmacist with relevant clinical experience.
  • Strong communication and interpersonal skills.
  • Proven ability to work collaboratively within a multi-disciplinary team.
  • Motivated and passionate about delivering excellent service within general practice.
