Summary:

We are seeking a highly motivated and experienced Clinical Pharmacist to join our dynamic team at NHS Jobs. The successful candidate will play a pivotal role in providing exceptional pharmaceutical care to patients, contributing to person-centered medicines optimization, and enhancing overall clinical care.

Responsibilities:

  1. Clinically assess and treat patients using expert knowledge of medicines in specific disease areas.
  2. Provide specialist expertise in the use of medicines, including information on side-effects and dosage.
  3. Conduct pharmaceutical medication reviews and offer health education advice through various channels.
  4. Lead person-centered medicines optimization and quality improvement initiatives.
  5. Contribute to public health campaigns, including flu vaccinations, childhood immunization, and adult immunization programs.
  6. Process specialist treatment and hospital medication discharge letters.
  7. Act as a key contact for all medication-related queries.
  8. Support practice-based audits and implement changes to improve clinical care.
  9. Conduct care planning for mental health, admission avoidance schemes, and end-of-life patients.
  10. Review daily pathology results and take appropriate actions.
  11. Undertake minor ailments triage and direct patients based on clinical need.
  12. Capture relevant patient health information and contribute to the quality outcomes framework.
  13. Monitor patients with complex, long-term conditions, ensuring optimized medication management.
  14. Provide clinical advice and expertise on medication.
  15. Establish ongoing professional relationships with individual patients.
  16. Liaise with other healthcare providers for the safe transfer of care.
  17. Deliver on relevant aspects of the PCN Directed Enhanced Service (DES).
  18. Play a central role in shared-care protocols, clinical research, and liaison with specialist pharmacists.
  19. Participate in a professional clinical network and access appropriate clinical supervision.
  20. Support the PCN clinical director as necessary.
  21. Manage the repeat prescribing reauthorization process.
  22. Review prescribing against local and national targets and address any issues.
  23. Conduct structured medicines optimization reviews in care homes.
  24. Contribute to the reduction of medicine wastage within practices across the PCN.
