Summary:

We are seeking a dedicated Clinical Specialist Pharmacist to join our team at Older Peoples Mental Health Services (OPMHS). The role encompasses Cognitive Impairment and Dementia Services (CIDS) and Specialist Older Adult Mental Health Services (SOAMHS) within the community pharmacy setting. As a valued member of the West London NHS Trust pharmacy team, you will contribute to the delivery of high-quality patient-focused clinical pharmacy services.

Aims of the Role:

  • Assist in providing a high-quality patient-focused clinical pharmacy service to OPMHS.
  • Focus on medicines optimisation within OPMHS.
  • Ensure community teams comply with medicine legislation, ensuring safe and secure handling of medicines.
  • Undertake medicines reconciliation when applicable.
  • Participate in multidisciplinary team meetings for community services.
  • Provide medicines information to OPMHS staff members, patients, and carers.
  • Contribute to the development, implementation, and training of staff in OPMHS on issues related to medicines optimisation and medicines safety.
  • Work with the Local Lead pharmacist and Band 7 pharmacists to conduct audits for service evaluation and improvement, aligned with West London NHS Trust and OPMHS directorate priorities.
  • Provide medication counseling and motivational interviewing to enhance medicines adherence for service users and their families.
  • Liaise directly with the Lead Older Adults Mental Health Pharmacist (8a) for clinical support.
  • Assist with other pharmacy responsibilities, including the dispense and supply of medications, accuracy checks, and support with other services as required by the pharmacy department.
