Summary: We are seeking a dedicated and experienced Clinical Pharmacist to join our team at NHS Jobs in Leicester. Key Responsibilities:
- Conduct medication reviews for patients with single or multiple medical problems, ensuring medicines optimization.
- Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement.
- Attend and refer patients to multidisciplinary case conferences, managing caseloads for common/minor ailments.
- Provide patient-facing clinics and a telephone help line for medication-related queries.
- Answer all medicine-related enquiries from GPs, practice staff, healthcare teams, and patients, suggesting and recommending solutions.
- Review and reconcile medicines post-hospital discharge and upon admission to care homes.
- Set up and manage systems to ensure continuity of medicines supply to high-risk patient groups.
- Contribute pharmaceutical advice for the development and implementation of new services with medicinal components.
- Analyze and interpret medicines data, presenting findings to support decision-making.
- Undertake simple audits of prescribing and contribute to national and local research initiatives.
- Implement changes to medicines based on MHRA alerts, product withdrawals, and other guidance.
- Monitor practice prescribing against local health economy’s RAG list and make recommendations to GPs.
- Assist practices in maintaining a practice formulary and auditing compliance against NICE technology assessment guidance.
- Provide newsletters or bulletins on important prescribing messages.
- Deliver education and training to primary healthcare teams on therapeutics and medicines optimization.
- Ensure compliance with CQC standards where medicines are involved.
- Support public health campaigns and provide specialist knowledge on all public health programs.
Qualifications and Skills:
- Degree in Pharmacy and registration with the General Pharmaceutical Council.
- Clinical pharmacy experience in a healthcare setting.
- Strong communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
- Commitment to continuous professional development.
