Summary

Join our team at SWYPFT as a Clinical Pharmacist for Maternity Cover. Contribute to a safe and effective specialist clinical pharmacy service within designated wards. Assist in overall pharmacy service provision, and deputize when required.

Role of Your Department

Develop efficient systems for procurement, compounding, and distribution of medicinal products.

Ensure compliance with current laws and Department of Health advice.

Provide professional advice on safe, cost-effective use of medicines.

Implement quality control mechanisms meeting medicines agencies’ requirements.

Address clinical risk issues through clinical governance and Medicines Management.

Responsibilities (Budgetary/Supervisory)

Coordinate clinical supervision and activities of pharmacists and technicians.

Oversee ward-based technicians and assistants.

Key Result Areas

Specialist clinical service provision to SWYPFT wards and departments.

Drug history taking, medication review, care plan formulation, and updating.

Therapeutic drug monitoring, patient education, and evidence-based treatment protocols.

Compliance aids and self-medication schemes facilitation.

Clinical audit, practice research, and reporting on interventions and outcomes.

Cost-effective prescribing promotion and professional supervision in dispensary.

Development of links with other professionals and recommendation of service changes.

Participation in reporting and monitoring medicines-related incidents.

Dispensing of clinical trials, clinical training, and participation in out-of-hours rotas.

Compliance with legal requirements, health and safety, data protection, and COSHH legislation.

Continuing Professional Development and adherence to national guidelines.

Contribution to Medicines Management strategy and pharmacy objectives.

Communications and Working Relationships

Internal: Pharmacy staff, doctors, nurses, dieticians, principal pharmacist, therapy staff, finance department, personnel department, estates and IT department, patients and their carers/families, administrative staff.

External: General practitioners, nursing and residential home staff, pharmaceutical representatives, NHS organizations, community pharmacists.
