Job Summary

The GP Care Group CIC is seeking a Primary Care Network (PCN) Clinical Pharmacist to join Network 1 PCN in Bethnal Green. This position offers the opportunity to work with a patient population of 50,000 across four practices, benefitting from the extensive support and employee benefits provided by the GP Care Group federation.


Main Duties of the Job

  • Medicine Management: Develop and implement an effective medicine management system within PCN practices.
  • Cost-Effective Prescribing: Work to achieve cost-effective prescribing and serve as an independent prescriber.
  • Support and Advice: Provide advice and support to both patients and colleagues.
  • Evolving Role: Adapt to the changing responsibilities and duties within the role over time.

Job Responsibilities

  • Clinical Medication Reviews: Conduct medication reviews with patients, provide recommendations, and support care home staff in improving medicine safety.
  • Patient Interaction: Manage caseloads of patients, provide patient-facing clinics, and offer a telephone help line for medicine-related queries.
  • Discharge Reconciliation: Reconcile medicines post-discharge from hospitals and care homes, ensuring continuity of supply.
  • Data Analysis: Analyse and present medicines data to highlight issues and support decision-making.
  • Audits and Implementation: Conduct clinical audits, implement changes based on MHRA alerts, and ensure compliance with NICE guidelines.
  • Education and Training: Provide education and training on therapeutics and medicines optimisation to primary healthcare teams.

Person Specification

Qualifications:

  • Essential: GPhC membership and an undergraduate degree in pharmacy.

Experience:

  • Essential: Understanding of the PCN Clinical Pharmacist role and its fit within primary care prescribing needs.

Skills and Abilities:

  • Essential: Completion or ongoing completion of the CPPE Primary Care Education Pathway.
  • Essential: Qualified as an independent prescriber or working towards the qualification.
  • Desirable: Interest in completing the advanced clinical pharmacist pathway.
