Summary

The Advanced Pharmacist will spearhead and manage medicines optimisation services and the development of clinical pharmacy in alignment with local, regional, and national strategies. The goal is to enhance the quality of care, personalize patient experiences, and ensure the delivery of high-quality and cost-effective prescribing.

Key Responsibilities

Clinical Leadership and Development

  • Provide clinical leadership to ensure high-quality, cost-effective prescribing practices.
  • Lead the planning, organisation, and coordination of medicines optimisation initiatives aimed at improving prescribing behaviours and patient outcomes, including reducing patient harm and enhancing medication safety.
  • Oversee the development and implementation of new medicines optimisation initiatives and services within GP practices, community pharmacies, and community providers.

Policy and Guidelines Development

  • Develop prescribing and medicines optimisation guidelines and protocols.
  • Work independently with considerable discretion, while collaborating with a wide array of internal and external stakeholders.

Communication and Information Management

  • Analyze and interpret complex information, and communicate it effectively to patients, carers, and healthcare professionals to promote safe and cost-effective use of medicines.
  • Share clinical information related to patient care, technical therapeutic information, and prescribing activity data derived from sources such as clinical audits and ePACT2.

Risk Management and Compliance

  • Provide specialized advice and support on risk management and clinical governance issues.
  • Ensure compliance with medicines legislation, patient safety advisory notices, and other established best practices to manage and minimize risk.

Professional Development

  • Maintain and develop professional competence and expertise.
  • Stay updated with the latest medical and therapeutic evidence, opinions, and policy developments at both local and national levels.
  • Establish personal development goals and participate in the appraisal process.
