Summary

Join our team as a Clinical Pharmacist and become an integral part of a multi-disciplinary team. This patient-facing role involves using your expertise to assess and treat patients, focusing on optimizing medication for specific disease areas. You will be responsible for managing the care of patients with chronic diseases and conducting clinical medication reviews, especially for elderly, frail patients, and those in care homes with multiple co-morbidities.

Key Responsibilities

Patient Care and Medication Management

  • Perform clinical assessments and treatments using your extensive knowledge of medicines.
  • Conduct medication reviews in various settings such as GP practices, care homes, and through domiciliary visits.
  • Target high-risk patients for medication reviews and provide recommendations to general practitioners.
  • Record medication plans and outcomes in clinical systems.

Leadership and Collaboration

  • Offer specialist expertise in medicine use and address public health and social care needs.
  • Lead person-centered medication optimization, ensuring antibiotics are used conservatively.
  • Support quality improvement initiatives and contribute to the quality and outcomes framework.
  • Attend multi-disciplinary team meetings and provide feedback.

System Integration and Development

  • Facilitate the integration of general practice with community and hospital pharmacy teams.
  • Collaborate with pharmacy professionals across various healthcare sectors.
  • Manage clinical aspects of shared care protocols and participate in clinical research.

Education and Training

  • Deliver training and mentorship to other clinicians and staff regarding medication issues.
  • Actively participate in peer supervision and regular team meetings.
  • Report and investigate medicine-related incidents and contribute to root cause analyses.

Quality and Compliance

  • Ensure compliance with CQC standards related to medication management.
  • Take responsibility for areas such as QoF, Quality Contract, and QIPP.
  • Promote cost-effective prescribing and provide leadership on prescribing systems.
