Summary

Job Role and Purpose

As a Clinical Pharmacist, you will play a crucial role in patient care, particularly focusing on long-term condition clinics. Your primary responsibility will be to conduct medicine optimization for patients with chronic conditions such as COPD and asthma. You will deliver key aspects of the Network DES, including conducting Structured Medication Reviews (SMRs) to ensure patients get the most benefit from their medications.

Key Responsibilities

Patient-Facing Long-Term Condition Clinics: Provide expert care to patients requiring medicine optimization, reviewing ongoing needs, monitoring requirements, and ensuring effective medicine use.

Structured Medication Reviews: Conduct thorough medication reviews, making appropriate recommendations for improvements to Senior Pharmacists or GPs.

Clinical Medication Review: Undertake clinical medication reviews and produce detailed recommendations for senior clinical pharmacists, nurses, and GPs.

Care Home Medication Reviews: Work closely with care home staff to enhance the safety and efficacy of medicines ordering and administration.

Domiciliary Medication Review: Perform clinical medication reviews for home-bound patients and collaborate with multidisciplinary teams to ensure comprehensive patient care.

Management of Common Ailments: Handle cases involving common or minor ailments, providing management within your scope of practice and competence.

Additional Duties

Attend and refer patients to multidisciplinary case conferences.

Collaborate with other healthcare professionals to ensure holistic patient care.

Continuously update your knowledge and skills to maintain high standards of pharmaceutical care.
