Job Summary
As a Pharmacy Technician, you will play a crucial role in supporting the Clinical Pharmacy team and Clinicians by providing both technical and administrative assistance. Your responsibilities will include facilitating medication changes under the supervision of the Clinical Pharmacist and conducting audits to ensure patient safety in medication prescribing. You will share audit findings with the clinical team to promote best practices.
Duties & Responsibilities
- Medication Management: Process repeat medication requests, including converting acute requests to repeat prescriptions and adjusting medication quantities as needed.
- Patient Interaction: Assist with the Pharmacy team’s telephone clinic to address patient medication queries.
- Prescription Issuance: Issue prescriptions following agreed clinical protocols and within your competency level.
- Medicines Reconciliation: Ensure accurate medication records post-discharge and during care transfers.
- Record Keeping: Maintain accurate patient medication records on the practice’s clinical computer system, documenting advice given and actions taken.
- Interdepartmental Coordination: Liaise with secondary and primary care colleagues to ensure medication continuity following care transfers.
- Policy Adherence: Monitor practice adherence to the repeat prescribing policy.
- Data Analysis: Assist the Pharmacist with data analysis of prescribing behaviors in line with NICE guidance and local clinical commissioning directives.
- Communication: Support the Prescribing team by sharing medication guidance and establishing effective communication with both the Dispensing team and Clinical staff.
- Audit Development: Assist in the development and review of medication audits.
- Chronic Disease Support: Help patients with chronic diseases make lifestyle changes to manage their conditions.
- Project Work: Engage in project work, prescribing initiatives, and clinical research under the Clinical Pharmacist’s support.
- Medication Queries: Act as a point of contact for medication queries from patients, clinicians, nursing teams, secondary care, and community pharmacists.

- Patient Interaction: Use sensitive communication styles to ensure patients are fully informed and consent to treatments.
- Effective Communication: Communicate effectively with patients and carers, recognizing the need for alternative communication methods based on different levels of understanding and cultural backgrounds.
