Job Summary
We are seeking a skilled and dedicated Practice Pharmacist to undertake clinical medication reviews with patients and provide recommendations. The successful candidate will work closely with GPs and other practice staff to support and develop high-quality, safe, cost-effective, rational, and evidence-based prescribing practices.
Main Duties of the Job
- Provide clinical care to support and improve access to services.
- Optimize the safety and effectiveness of medicine use, including prescribing where appropriate.
- Develop, implement, and evaluate processes to ensure successful implementation of NHS prescribing strategies and policies.
- Facilitate integrated, patient-focused care that meets the needs of the NHS population, in line with local and national strategies.
- Actively participate in and contribute to multi-disciplinary and multi-professional teams in a fully integrated manner.

Job Description
Job Responsibilities
- Utilize clinical and pharmaceutical knowledge to enhance patient care through activities such as complex polypharmacy reviews, long-term condition management clinics, medicines reconciliation following hospital discharge, and addressing acute prescription requests.
- Provide advice to GPs and other healthcare professionals on the safe and effective use of medicines.
- Take on the role of an independent prescriber to improve patient care through various activities, including complex medication reviews and managing common clinical condition clinics.
- Develop guidelines and protocols, manage long-term condition clinics, and conduct appropriate patient assessments.
- Offer specialist pharmaceutical advice on all aspects of prescribing to GPs, nurses, and other healthcare professionals.
- Review prescribing patterns and discuss recommendations with prescribers.
- Assist in achieving practice targets within the prescribing strategy.
- Address issues identified by Lead Locality Pharmacists through practice prescribing data analysis.
- Develop and implement practice-specific protocols to support cost-effective prescribing.
Person Specification
Qualifications
Essential:
- Diploma/MSc in Clinical Pharmacy or other relevant postgraduate qualifications or experience.
