Job Summary

Join our dynamic, innovative, and friendly team dedicated to enhancing our ward-based Clinical Pharmacy services. As a Senior Clinical Pharmacist, you will be an integral part of our multidisciplinary team, delivering high-quality Clinical Pharmacy services to our modern Admissions Units.

You will work Monday to Friday and participate in weekend (Saturday and Sunday) and Bank Holiday rotas.

Main Duties of the Job

As a dedicated and patient-focused Pharmacist, you will combine expert opinion and practice at the bedside to enhance patient care. You will work closely with the admissions team to implement, deliver, and continuously improve medicines management in the Admissions Units. Your responsibilities will include reconciling medications prescribed upon admission, conducting early in-depth medication reviews, and supporting the discharge process. This will ensure high-quality patient outcomes, reduce prescribing errors, and maintain patient safety.

You should have post-registration experience in hospital Pharmacy and have completed or be working towards a Post Graduate Diploma in General Pharmacy Practice or an equivalent qualification. Excellent communication, interpersonal, and time management skills are essential, as is the ability to work effectively within a multidisciplinary team.

The Pharmacy department at London North West Healthcare NHS Trust is a forward-thinking and supportive environment, offering comprehensive services including a respected Clinical Pharmacy service, specialist Clinical Pharmacists, and a Regional Medicines Information Centre. The department also features a large manufacturing unit. Northwick Park Hospital is home to one of London’s eight Hyper Acute Stroke Units (HASUs).

Qualifications:
Essential:

  • Pharmacy Degree
  • GPHC registered
  • Certificate in Pharmacy Practice

Desirable:

  • Post Graduate Clinical Diploma
  • Independent prescriber

Experience:

  • Hospital Pharmacy Experience

Desirable:

  • Clinical Pharmacy experience in an acute hospital setting
