- Leadership and Mentorship: Provide leadership, mentorship, and supervision for the Pharmacy team, including Pharmacists and Pharmacy Technicians, across practices within the network.
- Clinical Competence: Maintain clinical competence as a Prescriber, managing your own caseload, conducting clinical medication reviews, and proactively managing patients with complex polypharmacy, particularly frail elderly and vulnerable adults.
- Medicine Management: Support medicine reconciliation post-discharge, structured medication reviews, cardiovascular disease interventions, and clinical governance work. Ensure medicine optimisation, safety, antibiotic stewardship, and de-prescribing agendas are integrated into medication reviews.
- Collaboration: Work within a multidisciplinary team, integrating with wider healthcare teams, including Community and Hospital Pharmacy, and Intermediate Care colleagues. Communicate and implement medication changes as necessary.
