Summary: As a Highly Specialist Pharmacist in Medicine, you will play a pivotal role in providing clinical pharmacy services to medical patients. Your responsibilities will include reviewing prescriptions for accuracy, ensuring the effective supply of inpatient medications, and promoting safe and cost-efficient drug use. Additionally, you will collaborate with healthcare professionals, contribute to service development, and participate in research initiatives.
Key Responsibilities:
Clinical Practice:
- Review prescriptions for accuracy and legality, identifying potential issues.
- Ensure effective supply of medicines for inpatients and appropriate labeling for discharge.
- Monitor patients for potential adverse effects of medications.
- Support patient discharge planning with a focus on medication.
- Demonstrate professional accountability to patients.
- Liaise with the Lead Directorate Pharmacist for clinical pharmacy service development.
Leadership:
- Act as a role model for junior pharmacists, promoting best practices.
- Motivate and inspire others to improve care and create a positive working environment.
- Contribute to service development and clinical pharmacy services in line with objectives.
- Represent Pharmacy at multidisciplinary meetings and contribute to research initiatives.
Management of Service:
- Ensure national and local agendas are delivered.
- Lead responses to medication-related incidents within the directorate.
- Monitor and report on drug expenditure, develop and review clinical guidelines.
- Evaluate performance of junior staff and provide pharmaceutical and financial information.
Evaluation of Service:
- Monitor clinical pharmacy to ensure safe, effective, and cost-efficient drug use.
- Document workload and quality of pharmacy services in line with objectives.
- Evaluate pharmacy services for recognition in line with corporate and clinical governance goals.
Research and Service Development:
- Demonstrate a critical approach towards pharmacy practice.
- Undertake practice research and integrate findings into practice.
- Develop clinical pathways within the speciality area.
- Actively seek improvements using Quality Improvement Methodology.
Education and Training:
- Participate in training and supervision of junior pharmacists and pre-registration graduates.
- Identify training needs, develop strategies, and evaluate provided education and training.
- Contribute to clinical training of ward staff and maintain a CPD portfolio.
- Act as a GPhC pre-registration pharmacist Practice Supervisor/Educational Supervisor.
Other:
- Maintain a broad level of pharmacy practice, adhering to ethical and professional codes.
- Ensure responsive and inclusive communication, considering various recipients and potential barriers.
- Participate in education and training programs for continuous professional development.
- Adhere to relevant legislation and codes of conduct.
- Undertake any other duties reasonably required by the Chief Pharmacist.
