Job Summary
Key Responsibilities
Medicines Supply:
- Dispense and issue medications to patients using actual or faxed prescriptions.
- Provide advice to patients on effective and safe medication use.
- Handle payment for prescription charges and issue receipts when needed.
- Maintain accurate records for stock control and prescription issues using paper and computerised Pharmacy Information System (PIS).
- Coordinate with Procurement staff for urgent medication requirements and inform patients and healthcare staff about any supply issues.
- Dispense clinical trial medications and complete required documentation according to Standard Operating Procedures (SOPs).
- Handle specialist hospital-only and unlicensed medicines, ensuring detailed records.
- Dispense oral chemotherapy medications following SOP precautions.
- Replenish out-of-hours medicine cupboards, including A&E and Emergency drug cupboards, ensuring accurate record-keeping.
- Assist the lead Homecare Technician with the supply of medicines through Homecare Services, including order reviews and invoice reconciliation.
Medicines Management:
- Foster effective communication between Pharmacy and Ward staff for a safe, efficient, and cost-effective medicines management service.
- Work with nursing staff and ward Pharmacist on new admissions, discharges, and transfers to prioritize Pharmacy services.
- Respond to queries from nurses and other healthcare professionals, referring clinical issues to the Pharmacist.
- Support medicines management pharmacists in developing and maintaining care plans for ward patients, including:
- Assisting in taking accurate medication histories.
- Leading assessments and reviews of medicines brought in by patients.
- Educating patients on treatment changes and medication self-management where appropriate.
- Coordinating with PCTs, GPs, Community Pharmacists, and carers for seamless care through accurate transmission of medicines-related information.
- Ensuring the safety and appropriateness of medicines for patients at discharge.
- Collaborate with and train nursing staff on medicine management policies.
