Job Summary

Healthbridge Direct is seeking a dedicated Clinical Pharmacist to join the Loxford Primary Care Network (PCN). In this role, you will leverage your clinical expertise to manage medications, conduct face-to-face structured medication reviews, oversee long-term condition treatments, and improve prescribing safety. You will also handle repeat prescription authorizations, address acute prescription requests, and meet both the public health and social care needs of patients. Main Duties of the Job

As a Clinical Pharmacist, you will work within your clinical competencies as part of a multidisciplinary team. Your responsibilities will include:

  • Providing clinical expertise in medicines management.
  • Conducting face-to-face structured medication reviews.
  • Managing long-term conditions and medications during care transitions.
  • Ensuring safer prescribing practices.
  • Handling repeat prescription authorizations and reauthorizations.
  • Addressing acute prescription requests.
  • Meeting public health and social care needs in the GP practices within the PCN.
  • Performing medication reviews for patients with polypharmacy, particularly older adults, those in residential care homes, and patients with multiple comorbidities.

This role is crucial for enhancing care quality and operational efficiency, requiring a high level of motivation and passion for delivering excellent general practice service. Support for developing your role into a non-medical prescriber will be provided if this qualification is not already held.

Job Responsibilities

  • Perform medication reviews for patients with polypharmacy, especially older adults and those with multiple comorbidities.
  • Provide leadership on quality improvement and clinical audits.
  • Manage aspects of the Quality and Outcomes Framework.
  • Develop your role to become a non-medical prescriber, if not already qualified.

Person Specification

Qualifications

Essential

  • Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Understanding and adherence to relevant standards of practice.
  • Compliance with professional and organizational performance management policies/procedures.

Desirable

  • Clinical Diploma.

Experience

Essential

  • Minimum of two years of experience as a pharmacist, demonstrated in a practice portfolio.
  • Awareness of common acute and long-term conditions in general practice.
  • Ability to integrate general practice with community and hospital pharmacy teams.
  • Experience in partnership/collaborative working and building relationships across various organizations.

Knowledge and Skills

Essential

  • Minimum of 2 years post-qualifying experience.
  • Understanding of the wider determinants of health and their impact on communities.
  • Appreciation of primary care prescribing principles, rational prescribing, and strategies for improving prescribing.
  • Proficiency in IT systems for creating simple plans and reports.
  • Ability to analyze complex technical information and influence others to follow recommendations.
  • Skill in identifying and resolving risk management issues per policy/protocol.
  • Understanding of research governance principles.
