Summary: As a Pharmacy/Pharmacist Technician with NHS Jobs, you’ll play a vital role in ensuring the efficient and effective delivery of pharmaceutical services. Your responsibilities will include medicines reconciliation, supporting prescribing targets, contributing to the development of prescribing protocols, and participating in medication audits. You will advise on prescribing changes, monitor budgets, and provide essential support for medication reviews.

Job Responsibilities:

  • Undertake medicines reconciliation and update patients’ records.
  • Support the achievement of prescribing Quality and Outcomes Framework targets.
  • Contribute to the review and development of repeat prescribing protocols.
  • Provide advice to GPs, staff, and patients on prescribing changes.
  • Participate in medication audits and assist in managing prescribing budgets.
  • Prepare evidence-based resources for cost-effective prescribing.
  • Provide medication reviews, patient information leaflets, and community visits.
  • Proactively handle prescriptions, assess risks, and make improvement recommendations.
  • Respond to medication queries professionally, referring to GPs as per protocols.
  • Advise on the sourcing and safe management of medicines.

Responsibility for Administration:

  • Take messages and pass on information.
  • Perform computer data entry and processing in accordance with practice procedures.
  • Initiate contact and respond to requests from patients, team members, and healthcare professionals.
  • Maintain a positive, patient-focused and supportive culture within the practice team.

Patient Services:

  • Provide medication review services to patients in various settings.
  • Assist in monitoring and managing prescribing budgets.
  • Prepare evidence-based resources to support the medicine management team.
  • Help plan, develop, and support the introduction of new working processes for optimal prescribing quality.

Communication:

  • Communicate effectively within the team and with patients and carers.
  • Recognize alternative communication needs and respond accordingly.

Health & Safety:

  • Promote and maintain health, safety, and security according to practice policies.
  • Identify and manage risks in work activities.
  • Use personal security systems and infection control procedures.

Confidentiality:

  • Respect and uphold patient privacy and confidentiality.
