Job Summary
The PCN Pharmacy Technician position at Lakeside Healthcare is part of a transformative initiative driven by the NHS Long Term Plan. This role is critical to ensuring the safe and effective use of medicines within the Lakeside Healthcare system. Initially, the focus will be on medicines reconciliation from clinic and discharge letters, with responsibilities expanding as the Pharmacy team grows. Effective systems and procedures are essential for maintaining safe and efficient processes, and the selected candidate will play a key role in daily medication management and patient monitoring.
Duties and Areas of Responsibility
Medicines Reconciliation:
- Review clinic and discharge letters for medication updates.
- Central involvement in clinical aspects of shared care protocols.
Level 1 Medication Reviews:
- Identify patients on repeat medication lacking recent reviews.
- Check medical notes for necessary monitoring and arrange as needed.
- Update medications and remove outdated prescriptions as per protocol.
- Ensure dosage instructions are clear and concise.
- Align prescribing practices with the Lakeside Drug Formulary and CCG guidance.
Support and Liaison:
- Assist Clinical Pharmacists with general prescription requests and medication queries.
- Liaise with pharmacists on any concerns regarding patient medication regimens.
- Promote and implement electronic repeat dispensing.
- Conduct searches for audits and high-risk drug monitoring using clinical systems.
- Participate in practice and other relevant meetings to enhance the role of the pharmacy technician.
Training and Development:
- Engage in training and development programmes provided by Lakeside Healthcare.
- Participate in individual performance reviews and maintain records of personal and professional development.
- Collaborate with the Senior Pharmacist and Management team to assess and fulfill learning needs.
- Utilize learning opportunities and share insights with team members.
- Complete mandatory and statutory training as required.
Patient Safety and Optimisation Initiatives:
- Support antimicrobial stewardship efforts to reduce inappropriate antibiotic prescribing.
- Assist with the delivery of medicines optimisation schemes and audits related to MHRA alerts, drug monitoring, and patient safety.
Team Integration:
- Develop relationships with other pharmacy technicians, pharmacists, and multidisciplinary team members.
- Support the integration of the pharmacy team across health and social care settings, including primary care, community pharmacy, secondary care, and mental health.
