Job Summary

Symphony Healthcare Services Limited is seeking a dedicated Pharmacy Technician to join our Medicines Management Team for a fixed-term period of six months, working 30 hours per week. Ideal candidates will have prior experience in primary care settings.

You will work in one or both of our Medicines Management Hubs located at South Petherton Medical Centre and Creech Medical Centre. Collaborating with our team of pharmacists, pharmacy technicians, and prescription clerks, you will assist with medicines reconciliation, handle medication queries from patients and community pharmacies, and support all aspects of medication safety within the practice. Your role will involve liaising with GPs, practice staff, community pharmacies, and secondary care colleagues, ensuring patient safety and up-to-date drug monitoring checks.

Pay Rate: £13.99 – £16.32 per hour, depending on experience. Benefits include NHS or NEST Pension.

Main Duties of the Job

  • Support the practice pharmacist with medicines reconciliation and update patient medical records and prescriptions in accordance with practice protocols.
  • Conduct medication switches, dose optimization, and medicines synchronization to ensure safe and cost-effective prescribing.
  • Initiate repeat prescribing (eRD) for suitable patients.
  • Provide advice to GPs, staff, and patients regarding changes in prescribing to enhance safety, quality, and cost-effectiveness.
  • Participate in medication audits and QOF quality improvement projects.
  • Assist practices in meeting QOF indicators.
  • Ensure patient safety through high-risk drug monitoring and other safety scorecard indicators.
  • Respond professionally to medication queries from patients and staff via AskMyGP, phone, or email, referring to the appropriate clinician as necessary.
  • Advise on the sourcing and safe management of medicines.
  • Action drug alerts.
  • Provide information on KPIs to demonstrate the benefits of the role.
  • Act as a liaison between the practice and local community and hospital pharmacy services.
  • Support the prescription clerk team in resolving medicine-related queries.
  • Assist medicines optimization assistants and pre-registration pharmacy technicians in their professional development.
