We are seeking a dedicated Pharmacist to join our team at West London NHS Trust. The successful candidate will be responsible for collating activity and outcome data for their respective areas, and actively developing, monitoring, and integrating policies, guidelines, and procedures related to pharmaceutical care and medicines management within the service.

Key Responsibilities

  • Data Management: Collate activity and outcome data for designated areas.
  • Policy Development: Actively develop, monitor, and integrate policies, guidelines, and procedures covering all aspects of pharmaceutical care and medicines management.
  • On-Call and Weekend Service: Participate in the out-of-hours on-call service and weekend morning service on a rota basis, with additional payments provided for these shifts.
