Summary We are seeking a dedicated Pharmacy Technician to join our team at NHS Jobs in Darlington, DL3 7PL. As a Pharmacy Technician, you will play a crucial role in medicines reconciliation for patients recently discharged from hospitals. Your responsibilities will include ensuring accurate recording of new medicines on the GP clinical system, updating GP systems with secondary care-initiated medications, and fulfilling medication monitoring requirements.

Responsibilities

  • Undertake medicines reconciliation for discharged patients.
  • Monitor creatinine clearance for DOACs and verify correct doses.
  • Collaborate with the PCN for Quality Improvement Projects in medicines management.
  • Address medication queries from GPs, clinicians, and practice staff.
  • Manage care home medication requests and advice.
  • Update dose instructions and ensure patient understanding of new medications.
  • Work independently and within a team, managing competing priorities.
  • Contribute to reduced GP workload and medication costs.
  • Enhance patient safety, medication optimization, and outcomes.

Qualifications

  • Pharmacy Technician NVQ Level 3 or equivalent.
  • Good general education.
  • Satisfactory Disclosure and Barring check.
  • Completion of CPPE pharmacy technician pathway or willingness to undertake suitable training.
  • Registration with the General Pharmaceutical Council.
