Summary: Join our team at NHS Jobs as a Pharmacy Technician, contributing to the well-being of patients through effective medicines use and shared decision-making conversations. This permanent, full-time position is based in Wirral, CH49 0AB.
Duties and Responsibilities:
- Undertake patient-facing and patient-supporting roles to ensure effective medicines use, engaging in shared decision-making conversations with patients.
- Conduct medicines optimization tasks, including checking inhaler technique, supporting medication reviews, and medicines reconciliation.
- Utilize consultation skills to work in partnership with patients, ensuring effective use of medicines as required by the PCN.
- Support medication reviews and medicines reconciliation for new care home patients, synchronizing medicines for patient transfers between care settings, and collaborating with local community pharmacies.
- Assist the Clinical Pharmacist in Structured Medication Reviews (SMR), organizing necessary monitoring tests prior to SMR.
- Provide expertise to address public health and social care needs, offering lifestyle advice, service information, and assistance in tackling local health inequalities.
- Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.
- Support the PCN in delivering on QOF, IIF, and other locally commissioned enhanced services.
Requirements:
- Qualified Pharmacy Technician with relevant certifications and registration.
- Proven experience in patient-facing roles and medicines optimization tasks.
- Strong organizational and communication skills.
- Ability to work collaboratively with patients, healthcare professionals, and community pharmacies.
- Familiarity with medication reviews and reconciliation processes.
- Knowledge of public health and social care needs.
