Summary:

Take professional responsibility for the provision of a comprehensive clinical pharmacy service to designated medical specialities. This includes regular reviews of medicine prescriptions and reconciliation to ensure accuracy, legality, safety, and efficacy. Contribute significantly to divisional and departmental operational and strategic priorities, modernizing pharmacy services. Provide evidence-based advice on medications, attend ward rounds and clinics, and offer Therapeutic Drug Monitoring services to ensure product safety. Utilize non-medical prescribing skills to support medicine optimization practices and collaborate with community colleagues for seamless patient care transitions.

Key Responsibilities:

  1. Develop and review policies, procedures, and guidelines for medicine use within designated medical specialities.
  2. Work with CBU leads to support formulary applications and amendments.
  3. Provide pharmacy support at clinical meetings for effective alignment with medicine policy and practice.
  4. Lead in the development of medicine policies, procedures, guidelines, and Patient Group Directions.
  5. Identify and undertake medicine reviews and clinical audits within the CBU, reporting results to relevant committees.
  6. Report operational and strategic developments to the Principal Pharmacist Medicine Division.
  7. Manage recruitment, induction, development, and retention of pharmacy staff within the line management structure.
  8. Ensure implementation of Trust and national medicines policies and guidance.
  9. Lead the implementation of innovative ways of working through transformational change.
  10. Ensure accuracy, legality, efficacy, and safety of prescribed medication through clinical assessment and final checks.
  11. Participate in departmental work during weekends, public holidays, extended hours, and on-call as necessary.
  12. Work cooperatively with the Chief Pharmacist to foster an environment that supports staff engagement and decision-making.
  13. Deputise for the Principal Pharmacist Medicine Division when required, taking responsibility for management roles.

Qualifications:

  • Advanced pharmacy degree
  • Non-medical prescribing qualification
