Summary

Join our team at NHS Jobs as a Pharmacy Care Coordinator. Your role involves aligning with PCN priorities, supporting Quality and Outcome Frameworks, and coordinating health and care services. Collaborate with the PCN Pharmacist, ensuring service delivery aligns with PCN needs. Facilitate structured medication reviews, care homes reviews, and pharmacist visitation services. Proactively engage with high-risk patients, manage caseloads, and coordinate care across network practices. Enhance patient and carer experiences through effective communication and support.

Responsibilities

Coordinate and navigate health and care services in line with PCN priorities.

Liaise with PCN Pharmacist to set and achieve annual targets reflecting PCN DES and clinical goals.

Support structured medication and care homes review services.

Coordinate care homes pharmacist visitation service and engage with care home staff.

Encourage and arrange care for housebound patients, ensuring follow-up services.

Develop and run clinical searches to identify patients for pharmacist intervention.

Liaise with care coordinators at other network practices to share best practices.

Identify and work with high-risk patients, ensuring engagement with primary care services.

Maintain regular communication with the PCN pharmacist regarding patient progress.

Act as the first point of contact for caseload patients across network practices.

Coordinate referrals and carry out searches on EMIS for patient recall.

Support patient/carer contact roles and collect feedback on PCN services.

Assist patients in managing their needs, answering queries, and providing support.

Develop the role of Pharmacy Care Coordinator within the practices.

Maintain engagement with practice staff and encourage best practices.

Support national screening programs to increase local uptake.

Complete specified training requirements.

Other Duties

Take messages and pass on information.

Perform computer data entry and processing in accordance with PCN procedures.

Initiate contact and respond to requests from patients, team members, and healthcare professionals.

Help maintain a positive, patient-focused and supportive culture across the PCN team.

Communication

Communicate effectively with team members, patients, and carers.

Recognize patient needs for alternative communication methods and respond accordingly.
