Summary
As a Senior Pharmacist, you will play a pivotal role in providing specialized clinical pharmacy services to allocated wards and teams. This includes offering expert advice, leading clinical and patient services, and managing medicines-related projects.
Core Duties
- Specialized Clinical Pharmacy Advice: Attend multidisciplinary team meetings, offer specialized advice, and lead clinical services for assigned wards and teams.
- Audit and Quality Improvement: Identify and conduct audits, encourage multidisciplinary clinical audits, and support continuous improvement in pharmacy services.
- Dispensary Support: Assist with dispensing and checking prescriptions, and provide pharmaceutical advice to patients and staff both in-person and via telephone.
- Liaison with Healthcare Professionals: Work closely with medical and nursing staff to ensure the effective provision of pharmaceutical services.
- Patient-Centered Care: Offer specialist advice tailored to individual patient needs, review prescriptions, check dosages, and manage co-existing conditions.
Clinical Pharmacy Services
- Specialist Advice and Prescription Review: Provide comprehensive reviews of prescription charts, ensuring compliance with Trust procedures and addressing specific patient needs.
- Risk Management: Ensure compliance with medicines legislation and participate in risk management activities.
- Multidisciplinary Collaboration: Engage in care plan approaches, consultant ward rounds, and team meetings.
- Service Development: Contribute to the planning and development of the Clinical Pharmacy Service, ensuring cost-effective use of medicines.
Management Role
- Reporting and Evaluation: Prepare and present medicines usage and expenditure reports, identify cost-saving opportunities, and make recommendations for managing the medicines budget.
- Team Collaboration: Participate in pharmacy team meetings, support safe systems of work, and provide pharmaceutical advice to healthcare professionals, patients, and carers.
