Summary
The PCN Pharmacist role at NHS Jobs encompasses a range of responsibilities aimed at optimizing patient care and medication management. Key duties include:
Patient Facing: Conducting Clinical Medication Reviews.
Quality Improvement: Enhancing medicines quality through proactive measures.
Ailment Management: Addressing common, minor, and self-limiting ailments.
Telephone Support: Providing medicines support via telephone.
Care Home Setting: Managing medicines in a care home environment.
Long-Term Conditions: Overseeing medications for long-term conditions.
Prescribing Safety: Reviewing safer prescribing and responding to safety alerts.
Quality Outcomes: Contributing to the achievement of QoF/DES/IIF outcomes.
