Summary:

We are currently seeking a dedicated Clinical Pharmacist to join our dynamic team at NHS Jobs. This is a permanent, full-time position based in Canterbury, CT1 1AZ.

Responsibilities:

The successful candidate will be responsible for:

  • Developing close working relationships with other members of the multi-disciplinary team.
  • Contributing to clinical care plans by providing valuable insights into medication management.
  • Responding to medication-related queries and offering expert advice on prescribing practices.
  • Promoting evidence-based practice to ensure optimal patient care outcomes.
  • Engaging with patients and their carers to discuss medications, providing necessary information and support.

  • Educating colleagues about medicines management and promoting medicines optimization.

Qualifications and Skills:

To be successful in this role, you should possess:

  • A valid Pharmacist qualification and registration with the appropriate regulatory body.
  • Proven experience in a clinical pharmacy setting.
  • Strong communication and interpersonal skills.
  • The ability to work effectively within a multi-disciplinary team.
  • A commitment to promoting evidence-based practice and continuous improvement.
