Job Summary

The Clinical Pharmacist role entails providing clinical pharmacy services to the prisoner population, offering specialized advice and expertise to clinicians and nursing staff. The primary focus is on ensuring the safe, effective, and economical use of medicines within the prison healthcare system. Additionally, this role involves mentoring and training other pharmacists, contributing to the development of Medicines Management, participating in audit activities, and providing clinical pharmacy support in dispensing. Collaborating with locality teams and prison health teams is crucial to delivering an integrated service across the health community.

Main Duties of the Job

Provide specialized pharmaceutical care to medical patients, demonstrating expertise in therapeutics and medicines usage.

Serve as a clinical role model for junior pharmacists, ensuring safe and cost-efficient drug usage.

Offer a specialized clinical pharmacy service to the prisoner population at HMP Swansea, aligning with clinical pharmacy standards and objectives.

Assess patients’ pharmaceutical needs, develop care plans, and prioritize interventions both within the hospital and upon discharge.

Pharmacists registered as prescribers may prescribe within their competence level to support patient care.

Critically evaluate evidence and guidelines to formulate appropriate patient pharmaceutical care plans.
