Summary

NHS Jobs is seeking a dedicated and professional Clinical Pharmacist to join our team in Grantham. As a pivotal member of the multi-disciplinary team, you will work in a patient-facing role, managing chronic disease and conducting clinical medication reviews for patients with complex medication regimens. Your role will involve direct collaboration with the Pharmacist and Dispensary team, enhancing the quality of care provided to our patients.

Key Responsibilities

  • Chronic Disease Management: Take responsibility for managing patients with chronic conditions, ensuring they receive appropriate and effective treatment.
  • Clinical Medication Reviews: Conduct thorough medication reviews to manage complex polypharmacy, optimizing patient outcomes.
  • Prescription Support: Assist general practice staff with prescription and medication queries, support the repeat prescription system, and manage acute prescription requests.
  • Medicines Reconciliation: Oversee the reconciliation of medications when patients transfer care settings, ensuring safe and effective prescribing practices.
  • Clinical Leadership: Provide expertise in clinical medicines advice, drive medicines optimization, and contribute to quality improvement initiatives.
  • Community Integration: Facilitate the integration of practice with community and hospital pharmacies to enhance skill mix utilization, improve patient outcomes, and increase healthcare access.
  • Quality and Outcomes Framework: Manage aspects of the quality and outcomes framework, contributing to enhanced services and better care delivery.
  • Non-Medical Prescribing: Receive support and training to develop into a non-medical prescriber if required.

Essential Skills and Qualifications

  • Professional registration as a pharmacist.
  • Experience in a clinical setting, particularly within general practice.
  • Strong understanding of chronic disease management and polypharmacy.
  • Excellent communication and teamwork skills.
  • Ability to provide clinical leadership and drive quality improvement.
