Summary
Primary Duties and Areas of Responsibility
Lead medicines optimisation within the designated locality.
Provide clinical pharmacy service to the Intermediate Care Team.
Offer clinical pharmacy support to community hospital beds, collaborating closely with local General Practitioners.
Establish strong relationships with pharmacists in General Practice, Community Pharmacies, and Secondary Care to promote joined-up medicines optimisation.
Focus on supporting patients with polypharmacy and those frequently admitted to secondary care.
Provide Medicines Optimisation service to Care Homes in line with the latest guidance.
Offer timely, accurate information and advice to clinicians, nurses, health professionals, and the public to influence prescribing and ensure effective use of medicines.
Participate in multi-disciplinary groups to contribute to the safe selection, control, and administration of medicines.
Ensure effective implementation of medicine safety recommendations from regulatory bodies.
Co-ordinate and implement appropriate action following Safety Alert Broadcast System (SABS) recommendations across the Community Unit.
Participate in training and development initiatives.
Encourage an environment valuing all staff, promoting openness, critical evaluation of practices, and continuous improvement.
Provide plans for the continuous development of pharmacy services aligned with organizational objectives.
Develop systems for quality improvement, eliminating inappropriate variation, and continuously improving processes and outcomes.
Work proactively with stakeholders to improve pharmacy services continuously.
Incorporate user views in planning and delivering pharmacy services.
Manage internal and external complaints or incident reports related to pharmacy services efficiently.
Undertake investigations, identify root causes of suboptimal performance, and provide necessary reports.
Achieve agreed-upon objectives for personal management and development to contribute to improved pharmacy services and personal and professional development.
Liaise actively with relevant staff to achieve objectives in all key result areas.
Operate within current health and safety legislation, complying with departmental safety policies and procedures.
Strictly observe patient confidentiality and comply with Data Protection Act procedures.
Represent the service professionally in conduct, manner, and appearance.
