Job Summary

The Pharmacy Technician will play a critical role within the LSMP & TLS PCN Pharmacy Team, operating under the guidance of the Senior Clinical Pharmacist. This position ensures the safe, accurate, and timely provision of prescribed medications to patients. Key responsibilities include offering technical and administrative support to clinical pharmacists and clinicians, as well as contributing to the implementation of the Structured Medication Review and Medicines Optimisation DES specification.

Key Responsibilities

  • Medication Supply Management: Ensuring the precise and prompt supply of prescribed medications to patients.
  • Technical and Administrative Support: Assisting clinical pharmacists and clinicians with their daily tasks.
  • Structured Medication Review: Contributing to the implementation of the DES specification for medication reviews and optimization.
  • Medication Queries and Reconciliation: Addressing medication-related queries, managing acute prescription requests, and performing medicines reconciliation post-discharge from other care providers.
  • Patient Safety Audits: Conducting audits to ensure patient safety in medication prescribing, and presenting findings to the clinical team.
  • Liaison and Coordination: Working closely with patients, clinical and non-clinical staff, practice and community pharmacists, hospital staff, and patient carers to enhance pharmaceutical care.
