Summary

Primary Care Network Clinical Pharmacist

The Primary Care Network (PCN) Clinical Pharmacist will support East Lindsey GP Practices by implementing effective medicine management strategies, identifying areas for improvement, and driving change as part of the NHS England Clinical Pharmacists in General Practice pilot.

Patient Services

  • Manage repeat prescriptions, synchronize medication, and review discharge and clinic letters.
  • Address urgent script requests and conduct face-to-face reviews for complex medication regimes.
  • Collaborate with hospital pharmacists and conduct annual medication reviews for patients with long-term conditions.
  • Conduct medication reviews and de-prescribing in nursing, care, and residential homes.
  • Provide consultations in clinics and as requested by doctors, both in practice and domiciliary settings.

Administration

  • Provide regular support and feedback to practices on prescribing action plans.
  • Maintain accurate patient medication records and update the clinical team on prescribing policies.
  • Complete Clinical Commissioning Group (CCG) actions and disseminate prescribing tips to the team.

Communication

  • Communicate effectively with team members, patients, and carers.
  • Recognize and respond to alternative communication needs.
  • Report to Practice Managers, GP Partners, and the Senior Clinical Pharmacist Supervisor.

Confidentiality

  • Handle patient and business information with the utmost confidentiality.
  • Adhere to practice policies regarding the protection of personal and sensitive data.

  • Promote and manage health, safety, and security according to practice policies.
  • Ensure compliance with infection control procedures and maintain a clean and safe working environment.
  • Undertake regular health and safety training and manage the training of others.
