Summary: We are currently seeking a dedicated and qualified Clinical Pharmacist to join our dynamic clinical pharmacy team. The successful candidate will be responsible for a range of core responsibilities, working within the member PCNs practices and contributing to the improvement of medication services. The Clinical Pharmacist will collaborate with various stakeholders to ensure the highest standards of patient care.

Core Responsibilities: The Clinical Pharmacist will undertake the following responsibilities:

  • Provide medication review services with patients in member PCNs practices and during domiciliary visits to local nursing homes.
  • Conduct targeted medication reviews for high-risk patients.
  • Support and advise the PCN Pharmacy Team technicians and other PCN staff members on medication issues.
  • Practice as an independent prescriber within the scope of practice, if qualified.
  • Review and implement safe prescribing systems for vulnerable patients, improving compliance.
  • Conduct face-to-face and telephone consultations with patients with long-term conditions.
  • Support practice teams in understanding wider system issues (e.g., alerts, OTC, self-care, and homely remedies).
  • Administer influenza and other vaccines, if qualified.
  • Provide subject matter expertise on medication monitoring, implementing, and embedding a system.
  • Attend multi-disciplinary team meetings to provide advice on medication use and prescribing, receive referrals, and raise concerns.
  • Partake in clinical audits and contribute to their design.
  • Assist with the achievement of Investment and Impact Fund Targets.
  • Actively signpost patients to the correct healthcare professional.
  • Act as the practice point of contact for medicines-related matters, establishing positive working relationships.
  • Support clinicians with the management of patients suffering from prescription drug dependencies.
  • Completion of the CPPE training pathway Primary care pharmacy education pathway, unless completed previously or equivalent course.

Additional Responsibilities (May be requested to):

  • Provide education on medicines for the clinical team.
  • Assist with QOF target achievement for practices.
  • Review the latest guidance, ensuring practice conforms to NICE, CQC, etc.
  • Encourage cost-effective prescribing within the practice.
  • Provide advice and assistance to practices for them to accomplish work related to the Medicine Management Scheme to improve effectiveness and quality in prescribing and QIPP.
  • Advise and assist practices in implementing and embedding a robust repeat system.
  • Identify patients suitable for repeat prescribing, liaising with relevant clinicians as required.
  • Reconcile medicines following patients’ discharge from hospital or clinic appointments.
