Summary

Key Responsibilities

Conduct clinical structured medication reviews.

Manage patients with common/minor/self-limiting ailments within your scope of practice.

Offer advice to the Prescription Admin Team and address patient inquiries.

Reconcile medicines following discharge, identifying and rectifying changes.

Set up systems for continuity of medicines supply to high-risk patients.

Refer patients to appropriate healthcare professionals for timely care.

Support repeat prescribing reauthorization and identify high-risk patient cohorts.

Contribute pharmaceutical advice for the development of new services.

Analyze and present medicines data to support decision-making.

Undertake clinical audits of prescribing and implement changes.

Monitor practice prescribing against local health policies and guidance.

Provide newsletters and bulletins on important prescribing messages.

Deliver education and training on therapeutics and medicines optimization.

Ensure compliance with CQC standards where medicines are involved.

Support public health campaigns and provide specialist knowledge.
