Summary

The Clinical Pharmacy Technician will collaborate with ward pharmacists to implement medicines optimisation initiatives in line with departmental policies. Responsibilities include completing the medicine reconciliation process, assessing Patients Own Drugs (PODs), and ordering medicines as an accredited medicines optimisation pharmacy technician. Additionally, the role involves supporting the implementation of self-administration by patients and/or carers and managing the medication supply pathway efficiently, including the transfer of medicines between wards. The Clinical Pharmacy Technician will actively participate in patient counseling, obtaining specific accreditations for oral chemotherapy counseling, and liaise with ward and pharmacy staff to address technical queries or problems with patients, referring to the ward pharmacist when necessary.
