About the Role

The Clinical Pharmacist will provide essential support to general practice staff concerning prescription and medication queries. This role involves managing repeat prescriptions, handling acute prescription requests, reconciling medicines during care transitions, and ensuring safer prescribing practices. The pharmacist will offer clinical medicines advice, addressing both public and social care needs within the GP practice.

Main Duties of the Job

Patient-Facing Long-Term Condition Clinics

  • Manage patients with single or multiple medical issues requiring medicine optimization.
  • Deliver pharmacy-related aspects of the Network DES/LES, including conducting structured medication reviews.

Clinical Medication Reviews

  • Perform medication reviews and provide recommendations for senior pharmacists, nurses, or GPs on prescribing and monitoring.

Medicines Management

  • Assist with developing and managing the repeat prescribing process.
  • Support the reauthorization process by reviewing patient requests for repeat prescriptions and medicines reaching review dates.

Patient Support

  • Provide telephone support for medicine-related inquiries from GPs, practice staff, healthcare teams, and patients.
  • Assist with medicines management at hospital discharge, ensuring continuity of supply and proper reconciliation.

About Us

Island Health is the first GP surgery to become a limited company, with a growing patient list of over 15,500. The practice includes three directors, four partners, a management team, a lead nurse, a trainee nursing associate, an HCA, and a phlebotomist. Our admin team is 13 strong.

Job Description

Key Responsibilities

  • Work within professional boundaries as part of a multidisciplinary team.
  • Manage chronic disease areas within the practice and perform clinical medication reviews for patients with complex polypharmacy.
  • Provide clinical leadership on medicines optimization and quality improvement.
  • Integrate with community and hospital pharmacy to enhance patient outcomes and healthcare access.
  • Conduct patient-facing clinics for those with medicines queries and provide telephone support.
  • Assist with the development and management of the repeat prescribing process.
  • Identify and manage patients at high risk of harm from medicines.

Education and Training

  • Provide training to the primary healthcare team on therapeutics and medicines optimization.
  • Engage with Patient Participation Groups (PPGs) to develop the role further.

Person Specification

Qualifications

  • Master’s degree in pharmacy (MPharm) or equivalent.
  • Completion of the 18-month CPPE pathway (or equivalent) or actively working towards it.
  • Independent prescriber (or actively working towards it).

Experience and Skills

  • Prior experience in GP/clinical pharmacy.
  • Competent in managing multiple chronic diseases and able to independently prescribe.
  • Strong analytical skills and ability to manage pharmaceutical care programs.
  • Proficiency in legal, ethical, professional, and organizational policies.
